A briefing document has been published to advise nurses on personalised care.
The guidelines were issued by the NHS Confederation and Social Care Institute for Excellence.
The body said the move will strengthen the relationships between health professionals and local authorities.
It said this will be done by rolling out systems and procedures to "help integration" between the groups.
The move will also see the role of some health professionals undergo changes.
SCIE workforce director, Stephen Goulder, said: "Personalisation may sometimes mean staff looking at health services in an entirely different way."
